This privacy policy explains how your personal information is collected, stored and protected.

What is personal information?

Personal information is information collected from you in order to identify you and your child(ren) as past, present or future Monarch patients, including your name, address, email address, phone number, emergency contacts, personal health number (Carecard) medical history, dental records, insurance information, billing information and similar information about your child.

How does Monarch obtain your personal information?

Monarch only obtains personal information you give us:

  • on your new patient registration form

  • at one of our offices, or in communication with us via phone, email, social media or on our website

  • via referral from another health or dental care provider, if you have given them permission to share your information with Monarch on your behalf

 

How does Monarch use and share personal information?

Monarch will not, without your consent, use your personal information for any purpose other than as described in this privacy policy, except where permitted or required by applicable legislation.

Monarch will use your information to:

  • -Contact you regarding your child's dental health and/or treatment

  • -Remind you of appointments or discuss your account

  • -Bill your insurance directly if applicable

  • -Share your, or your child's x-rays with another dental professional

  • -Coordinate treatment with another health or dental professional

  • How do we protect personal information?

The security of your personal information is important to Monarch. For information provided online, our website contains security measures in order to protect against the loss, misuse, or alteration of the information under our control. Monarch's website host's servers are located in controlled and secure environments.

Under no circumstances will Monarch sell your personal information.

How can I update my information or unsubscribe?

At each appointment one or more Monarch family member will ask if there have been changes to your personal information. Monarch also encourages you to update any changes to your personal information or insurance information as soon as you know it via telephone, even if you do not have a pending appointment.

If you have subscribed to receive information, you may unsubscribe by clicking “Unsubscribe” at the bottom of the email message.

WHAT ABOUT ANTI-SPAM LEGISLATION?

Most of the messages Monarch may send you are exempt from the application of Canada’s anti-spam law as it relates to commercial electronic messages sent to electronic addresses. This is because: the electronic messages that Monarch may send are generally messages of a public service, not of a commercial character. If Monarch does send any messages to which the law applies and for which there are no exemptions, Monarch will ensure that we have consent to do so, as required by the law, and that any other legal requirements e.g. an unsubscribe mechanism, are met.

As a best practice, Monarch has an unsubscribe mechanism for electronic messages, even where the law does not require us to do so.

DOES THIS WEBSITE LOG IP ADDRESSES?

I log IP addresses, or the location of your computer network on the Internet, for systems administration and troubleshooting purposes only. I may also use IP addresses to track which pages people visit in order to improve the quality of our website.

DOES THIS WEBSITE USE COOKIES?

Like many websites, this website uses cookies which are small text files stored on the user’s browser. This website uses cookies to, for example, assist with site traffic analysis which includes tracking the time and date of website visits, pages viewed and referring URL’s. There are simple ways to refuse cookies, or accept them each time they are offered. Consult your browser’s online help for assistance on changing cookie settings or removing cookie files.

Office policy on media

From time to time Monarch may request to take a photo of you and/or your child in order to celebrate a successful visit or to use as promotional material on Monarch's media venues.  In the event that your child's image will be captured you will be specifically asked for consent and informed of Monarch's intentions. You reserve the right to refuse.

Due to the nature of technology today Monarch cannot ensure that other parents or guardians do not take photo's or videos in the office in which photo your child may appear in the background. Although un authorized photography or videography in the clinic is prohibited and Monarch makes every attempt to discourage it, Monarch is not responsible for photographs taken by other patients, parents or guardians.
